Job Description

Renewal Rehab is seeking a compassionate Physical Therapist Assistant to provide high-quality, evidence-based therapy to seniors in skilled nursing and post-acute care settings. In this role, you will implement PT treatment plans, deliver therapeutic exercises, gait and balance training, and functional mobility interventions to improve strength, safety, and independence. You will document progress in EMR, collaborate closely with an interdisciplinary team, and educate patients and families. Responsibilities Implement PT treatment plans for senior patients in skilled nursing and post-acute care settings Provide therapeutic exercises, gait training, and functional mobility interventions to improve strength, balance, and independence Accurately document treatments, patient progress, and outcomes in the EMR system Collaborate with Physical Therapists, nursing, and interdisciplinary team members to support patient-centered care plans Educate patients and families on home exercises, safety, and functional strategies to promote carryover Monitor patient response, ensure safety, and promptly communicate changes in condition to supervising PTContribute to quality improvement initiatives and uphold clinical best practices and regulatory standards Support a culture of integrity, respect, and open communication within the care team Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training Modalities application (e.g., heat, cold, e-stim) Care planning and documentation (EMR) Patient and family education Vital signs monitoring and safety awareness Interdisciplinary team collaboration Time management and caseload prioritization Knowledge of senior/geriatric care and fall prevention
