Job Description

Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support seniors in skilled nursing and post-acute care settings. In this onsite role, you'll deliver therapeutic exercises, gait and balance training, and functional mobility interventions under the direction of a Physical Therapist. You will document progress, collaborate with an interdisciplinary team, and help patients regain independence and quality of life. Responsibilities Provide therapeutic exercises, gait and balance training for senior patients under the supervision of a Physical Therapist Implement individualized treatment plans focused on restoring function, mobility, and independence in post-acute and long-term care settings Accurately document treatments, patient progress, and outcomes in the electronic medical record in a timely manner Collaborate with PTs, nursing, and interdisciplinary team members to support coordinated, patient-centered care Educate patients and caregivers on safety, home exercise programs, and strategies to maintain functional gains Adhere to all regulatory, ethical, and facility standards while promoting integrity, respect, and quality outcomes Participate in mentorship, in-services, and professional development opportunities offered by Renewal Rehab Required Skills Therapeutic exercise implementation Gait training and balance interventions Functional mobility training Patient assessment and documentation (EMR) Pain management modalities (e.g., heat, cold, TENS) Knowledge of geriatric conditions and precautions Care plan implementation under PT direction ROM (range of motion) techniques Infection control and safety practices Interdisciplinary team collaboration
