Job Overview Rehab Link is looking for licensed Physical Therapist Assistants to join our dedicated healthcare team as quality providers of Home Health services! In this vital role, you will deliver personalized, high-quality physical therapy care directly in patients' homes, empowering individuals to regain mobility, reduce pain, and improve their overall well-being. Your expertise will help foster independence and enhance quality of life for diverse patient populations, including geriatrics, pediatrics, and those recovering from acute or chronic conditions. Duties Implement individualized physical therapy treatment plans under the supervision of licensed physical therapists, ensuring adherence to clinical protocols and standards. Conduct thorough clinical assessments and document patient progress accurately in electronic health records (EHR), maintaining compliance with medical documentation standards. Apply specialized techniques such as trigger point therapy and manual therapy to alleviate pain and improve mobility. Collaborate closely with healthcare professionals, caregivers, and patients to develop comprehensive care plans tailored to each individual's needs. Educate patients and their families on exercises, safety measures, and discharge planning to promote ongoing recovery at home. Maintain clear communication with clients regarding treatment goals, progress updates, and any adjustments needed to optimize outcomes. Requirements Valid certification as a Physical Therapist Assistant with current licensure or registration as required by state law. Proven experience in home health settings, skilled nursing facilities, inpatient units, outpatient clinics, or similar environments—especially with geriatrics, orthopedics, or acute care. Strong knowledge of physiology, anatomy, medical terminology, and clinical assessment documentation standards. Proficiency in medical documentation practices and electronic health records (EHR) management. Skilled in manual therapy techniques such as trigger point therapy and other physiotherapy interventions. Excellent communication skills for effective caregiver and client interactions; ability to explain treatment plans clearly. Experience with discharge planning processes and collaboration with multidisciplinary teams for seamless patient care transitions.
