Responsibilities Assist licensed physical therapists in developing and executing comprehensive care plans tailored to each patient's needs. Provide hands-on therapy interventions such as therapeutic exercises, manual therapy techniques, and trigger point therapy to improve mobility and reduce pain. Monitor patient progress regularly, document treatment outcomes accurately in electronic health records (EHR) systems, and communicate updates to the healthcare team. Educate patients and their families on home exercise programs, safety measures, and community resources to support ongoing recovery. Collaborate effectively with healthcare professionals including physicians, nurses, and social workers to coordinate discharge planning and ensure seamless patient care transitions. Conduct assessments of patients' physiology, anatomy, and functional status to inform treatment adjustments. Maintain compliance with clinical documentation standards and adhere to all regulatory guidelines for home health services. Skills Proven experience working with geriatric patients in home health or outpatient rehabilitation settings. Strong knowledge of physiology, anatomy, orthopedics, neurology, pediatrics, and geriatrics to inform effective treatment strategies. Familiarity with outpatient clinic experience, inpatient hospital settings, or outpatient rehabilitation centers is highly desirable. Proficiency in medical terminology and physical therapy documentation practices within electronic health records (EHR) or electronic medical records (EMR) systems. Ability to develop and modify treatment plans based on patient progress and clinical assessments. Excellent client communication skills to educate patients about their care plans and motivate adherence. Experience with discharge planning processes and working collaboratively with multidisciplinary teams for optimal patient outcomes. Knowledge of outpatient rehabilitation techniques such as trigger point therapy and physiotherapy modalities.
