We act like owners We're looking for a PTA who brings these values into patient care and team collaboration. ️ Position Summary As a Home Health Physical Therapist Assistant (PTA) , you will work under the supervision of a licensed Physical Therapist to provide hands-on, one-on-one care to patients in their homes. This role will primarily serve patients throughout the east Portland metro area (east of the Willamette River) and offers autonomy, flexibility, and meaningful patient relationships . ✅ Key Responsibilities Patient Care Provide physical therapy treatments according to the plan of care established by the supervising PT Assist patients in improving: Mobility Strength Balance Functional independence Observe and report patient responses to treatment Collaboration & Care Coordination Work closely with Physical Therapists, nurses, and the interdisciplinary care team Communicate patient progress and recommend adjustments to care plans Participate in care coordination and case discussions Patient & Caregiver Education Instruct patients and families on: Home exercise programs Safety and fall prevention strategies Proper use of assistive devices Documentation & Compliance Complete accurate, timely clinical documentation Follow Medicare home health guidelines and Oregon state regulations Ensure compliance with company and regulatory standards Quality & Outcomes Support patient-centered outcomes and satisfaction Help reduce hospital readmissions and promote independence Qualifications Required Licensed Physical Therapist Assistant (PTA) in Oregon Graduate of an accredited PTA program Current driver's license and reliable transportation Ability to work independently in a field-based setting Preferred Home health experience Familiarity with OASIS and Medicare documentation Strong time management and clinical skills What Makes This Role Special ✅ Flexible scheduling and autonomy ✅ One-on-one patient care (no double-booking or clinic overload) ✅ Supportive leadership and strong team culture ✅ Opportunity to serve a consistent east side Portland territory ✅ Growth into leadership or advanced clinical roles ✅ Backed by Pennant's industry-leading training and leadership development programs Compensation & Benefits Pay Range $36.00- $42.
