Job Description
Tender Touch Rehab Services is seeking a compassionate Physical Therapist Assistant to support high-quality rehab services for seniors in skilled nursing and long-term care settings. In this on-site role, you'll carry out treatment plans designed by Physical Therapists, providing therapeutic exercises, gait and balance training, and functional mobility interventions that enhance residents' safety, independence, and quality of life. You will document progress in EMRs, partner with an interdisciplinary care team, and benefit from strong clinical support, mentorship, and growth opportunities within a collaborative, patient-focused culture. Responsibilities Implement physical therapy treatment plans under the direction of a licensed Physical Therapist Provide therapeutic exercises, gait training, balance activities, and functional mobility interventions to geriatric residents Assist with safe patient transfers, positioning, and use of assistive devices Accurately document treatments, patient responses, and progress in electronic medical records Communicate resident status and goals with Physical Therapists, nursing staff, and the interdisciplinary care team Educate patients and families on exercises, mobility strategies, and safety techniques Follow all infection control, safety, and regulatory guidelines within skilled nursing and long-term care settings Participate in care planning, team meetings, and quality improvement initiatives Support a compassionate, patient-centered environment that promotes dignity and independence Engage in ongoing professional development and training provided by Tender Touch Rehab Services Required Skills Therapeutic exercise implementation Gait training and balance interventions Modalities and manual therapy assistance Patient transfer and mobility techniques Electronic medical records (EMR) documentation Care planning and goal tracking Geriatric rehabilitation knowledge Infection control and safety procedures Interdisciplinary team collaboration Patient and family education
