Job Description
Tender Touch Rehab Services, a leading provider of therapy in skilled nursing and long-term care facilities, is seeking a Physical Therapist Assistant to support geriatric and sub-acute residents. In this role, you will implement individualized treatment plans under the direction of a licensed PT, focusing on mobility, strength, balance, and functional independence. You'll collaborate closely with an interdisciplinary team in a compassionate, patient-focused environment and benefit from strong clinical support, professional development, and growth opportunities in a respected rehabilitation organization. Responsibilities Provide physical therapy interventions under the supervision of a licensed Physical Therapist Carry out individualized treatment plans to improve residents' mobility, strength, balance, and functional independence Accurately document treatments, resident responses, and progress in the electronic medical record Assist with patient evaluations by performing delegated tests and measurements Educate patients, families, and caregivers on exercises, safety, and functional strategies Collaborate with nursing, rehab, and facility staff to support coordinated, high-quality resident care Ensure safe use of equipment and adherence to infection control and facility policies Participate in care conferences and contribute clinical observations to the interdisciplinary team Support a compassionate, resident-centered environment aligned with Tender Touch Rehab Services' values Engage in ongoing training and professional development offered by the organization Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training Transfer and wheelchair safety techniques Use of rehab equipment and modalities Electronic medical record documentation Care plan implementation under PT supervision Patient and family education Interdisciplinary team collaboration Observation and reporting of patient progress
