Job Description

Job description: Pinnacle Home Care, LLC is currently seeking a skilled and caring Physical Therapist Assistant (PTA) to join our home health team. Under the direction and supervision of a licensed Physical Therapist, you will deliver high-quality rehabilitative care, assist patients in achieving their mobility and functional goals, and ensure the best possible outcomes. PTA collaborates with Physical Therapist as well as other interdisciplinary team members regarding the care and treatment necessary to improve the patient's health condition.
Service Area:
This position will primarily serve patients in Northern Rhode Island, including: Woonsocket Cumberland Lincoln Burrillville Pascoag and surrounding communities
Responsibilities:
Make home visits to patients. Implements patient's individualized treatment plan as established by Physical Therapist. Work with other members of the rehabilitation team to implement programs consistent with the needs and capabilities of each patient Educate and instruct patients on activities necessary to promote their health, safety, and independent living. Accurately and timely document observations, interventions, and evaluations utilizing a touch pad tablet that is provided to you by the company.
Qualifications:
Current license to practice as a Physical Therapy Assistant in RI. Valid CPR license. Valid driver's license and proof of insurance coverage. Ability and willingness to work in a team environment. Ability to travel between patient assignments within a 25-mile radius.
