Job Summary We are seeking a dedicated and energetic Physical Therapist Assistant (PTA) to join our skilled nursing facility and long-term care (LTC/SNF) team. Duties Assist licensed physical therapists in implementing comprehensive physical therapy treatment plans tailored to each patient's needs. Perform therapeutic exercises, manual therapy techniques—including trigger point therapy—and modalities to facilitate recovery and improve function. Operate and maintain medical equipment safely and effectively during therapy sessions. Document clinical assessments, treatment progress, and discharge planning accurately in electronic health records (EHR) following clinical documentation standards. Collaborate closely with physicians, nurses, caregivers, and other healthcare professionals to ensure cohesive patient care and optimal outcomes. Communicate clearly with patients and their families about treatment goals, progress, and post-discharge care instructions. Conduct routine clinical assessments and update care plans based on patient response and evolving needs. Experience Prior inpatient experience in skilled nursing facilities (SNF), long-term care (LTC), or acute care settings is highly preferred. Demonstrated proficiency in physical therapy treatment implementation across diverse populations including geriatrics, pediatrics, orthopedics, and outpatient clinics. Hands-on experience with manual therapy techniques such as trigger point therapy and physiotherapy modalities. Strong knowledge of anatomy, physiology, medical terminology, and clinical assessment documentation standards. Familiarity with medical documentation practices within electronic health records (EHR) systems is essential. Proven ability to communicate effectively with clients, caregivers, and multidisciplinary teams to develop comprehensive care plans. Skilled in discharge planning processes to ensure seamless transition of care from facility to home or other settings.
