Job Description

Addus Family Company is seeking a compassionate Physical Therapist Assistant to support in-home rehabilitation services for elderly, chronically ill, and disabled clients. Working under the direction of a licensed Physical Therapist, you will implement treatment plans, assist with therapeutic exercises, mobility training, and use of assistive devices to help clients maintain safety and independence at home. You will document progress, report changes in condition, collaborate with nurses and caregivers, and promote compliance with therapy programs. This role offers ongoing training, clinical support, and growth opportunities in a mission-driven, patient-centered organization. Responsibilities Provide in-home physical therapy interventions under supervision of a licensed Physical Therapist Assist patients with therapeutic exercises, gait training, and mobility activities Educate patients and caregivers on home exercise programs and safe use of assistive devices Observe, measure, and document patient progress and response to treatment Report changes in patient condition to supervising Physical Therapist and clinical team Ensure treatments follow care plans, safety standards, and regulatory requirements Collaborate with nurses, caregivers, and other home care staff to support patient goals Maintain accurate, timely, and compliant clinical documentation Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility and transfer techniques Use and fitting of assistive devices Clinical documentation and EMR use Knowledge of home health and rehab protocols Patient and caregiver education Infection control and safety practices Time management and scheduling in the field Interdisciplinary care coordination
