Home Health Care What You'll Do Provide coordinated physical therapy to home-bound patients across various age groups under the supervision of a Licensed Physical Therapist. Actively participate in patient care coordination to help patients meet their recovery goals. Maintain clear documentation and adapt care to individualized patient needs.

What We're Looking For Education:

Associate degree from an accredited Physical Therapist Assistant program.

Licensure:

Active State PTA License (or multi-state license via the PTLC Compact).

Certifications:

Current CPR certification.

Experience:

1+ years of experience as a PTA preferred (Home Health experience a plus).

Skills & Requirements:

Strong interpersonal skills, ability to work independently, valid driver's license, and ability to handle physical demands (lifting, standing, local travel).
