Snap Health is seeking a dedicated Physical Therapist Assistant (PTA) . The PTA will provide skilled physical therapy services to patients in their homes under the supervision of a licensed Physical Therapist, helping patients improve mobility, strength, and functional independence.
Responsibilities:
Provide physical therapy treatments in accordance with the established plan of care. Carry out therapeutic exercises, gait training, balance activities, and functional mobility training. Monitor patient progress and report changes to the supervising Physical Therapist. Educate patients and caregivers on exercises and safety techniques. Maintain accurate and timely documentation of visits and patient progress. Communicate effectively with the interdisciplinary care team. Adhere to agency policies, procedures, and regulatory standards.
Qualifications:
Current Physical Therapist Assistant license in the State of Texas. Minimum 1 year of PTA experience preferred (home health experience a plus). Strong clinical, communication, and time-management skills. Ability to work independently in a home health setting. Valid driver's license, reliable transportation, and auto insurance. Proficient with mobile apps and electronic documentation systems.
