Job Description
Job Summary Join our dynamic healthcare team as a Physical Therapist Assistant, where you will play a vital role in delivering high-quality rehabilitative care to diverse patient populations. In this energetic and rewarding position, you will assist licensed physical therapists in implementing treatment plans, supporting patient recovery, and enhancing mobility. Your expertise will help patients regain independence and improve their quality of life through compassionate, skilled care. Duties Assist licensed physical therapists in developing and executing personalized treatment plans for patients in their home. Implement physical therapy treatments to promote healing and mobility. Conduct clinical assessments and document patient progress accurately following established documentation standards and medical terminology. Operate medical equipment safely and effectively during therapy sessions, ensuring proper use and maintenance. Collaborate with healthcare professionals such as physicians, nurses, and occupational therapists to coordinate comprehensive patient care plans. Maintain detailed electronic health records (EHR) to ensure seamless communication and compliance with clinical documentation standards using Wellsky/Kinnser software. Support discharge planning by preparing detailed reports on patient progress and recommending ongoing care needs. Skills Strong knowledge of physiology, anatomy, orthopedics, geriatrics, and related medical fields to inform effective treatment strategies. Excellent communication skills for engaging with patients, caregivers, and interdisciplinary team members to foster understanding and cooperation. Experience working in inpatient settings, skilled nursing facilities, outpatient clinics, or home health environments is highly desirable. Familiarity with medical documentation practices, clinical assessment documentation, and medical terminology to ensure accurate record keeping.
