Physical Therapist Assistant (PTA) -
South Creek Post Acute Part-Time:

We're looking for a Physical Therapist Assistant (PTA) who's passionate about helping residents restore mobility, confidence, and independence. General Purpose Under the direction of a licensed Physical Therapist, the PTA provides hands-on care, implements treatment plans, and helps residents reach their therapy goals in a positive, team-oriented environment. Key Responsibilities Deliver high-quality therapy services per the Physical Therapist's care plan Document daily treatments and weekly progress notes accurately and on time Support restorative nursing training and participate in team meetings Educate residents, families, and caregivers in safe mobility and exercise routines Maintain treatment areas and equipment in good working order Communicate effectively with the care team regarding progress and needs Qualifications Licensed Physical Therapist Assistant in Washington State Strong communication and teamwork skills Experience in skilled nursing or post-acute rehab preferred Familiarity with Casamba or PointClickCare a plus At South Creek Post Acute , we take pride in our positive culture, collaborative environment, and commitment to helping our residents thrive.
