Collaborate with healthcare professionals, caregivers, and patients to ensure cohesive care plans and effective communication. Support discharge planning processes by preparing patients for transition from inpatient or outpatient settings to home or other care environments. Adhere strictly to clinical documentation standards and medical terminology to ensure compliance with healthcare regulations. Requirements Certification as a Physical Therapy Assistant from an accredited program; licensure or registration as required by state or local regulations. Prior experience working in inpatient settings, skilled nursing facilities, outpatient clinics, or home health environments is highly desirable. Knowledge of physiology, anatomy, orthopedics, geriatrics, pediatrics, and related medical terminology. Proficiency in clinical assessment documentation, medical documentation practices, and electronic health records (EHR) management. Skilled in manual therapy techniques including trigger point therapy and other physiotherapy modalities. Familiarity with medical equipment operation relevant to physical therapy treatments. Strong communication skills for effective interaction with clients, caregivers, and multidisciplinary teams. Ability to work efficiently in fast-paced environments such as acute care or outpatient clinics while maintaining attention to detail.
