In this energetic and rewarding position, you will assist licensed physical therapists in implementing treatment plans, improving strength and balance deficets and enhancing mobility overall. Your expertise in therapeutic exercises, and clinical documentation will contribute to provide positive patient outcomes in an outpatient setting. Responsibilities Assist licensed physical therapists in developing and executing personalized care plans for geriatric patients living in independent, assisted and memory care. Implement physical therapy treatments such as therapeutic exercises to improve mobility, strength, and function. Operate medical equipment safely and effectively during therapy sessions, ensuring proper use and maintenance. Document clinical assessments and treatment progress accurately in a point of service in our web based EMR. Experience Prior outpatient experience in physical therapy is highly preferred. Demonstrated proficiency in clinical assessment documentation and medical documentation practices per Medicare guidelines. Hands-on experience with geriatrics is desirable. Familiarity with electronic health records (EHR) management systems to document patient progress efficiently. Strong communication skills for effective interaction with patients of diverse backgrounds and healthcare teams are vital for success in this role.
