Job Description
Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support high quality rehab services for seniors in skilled nursing and post acute settings. Working under the direction of a Physical Therapist, you will implement treatment plans, lead therapeutic exercises, document progress, and educate patients and families to improve mobility, safety, and independence. You'll collaborate closely with an interdisciplinary team in a supportive, patient centered culture that values integrity, respect, and open communication, with mentorship, clinical resources, and opportunities for professional growth. Responsibilities Implement PT treatment plans under supervision of a licensed Physical Therapist Provide therapeutic exercises, gait training, balance and functional mobility interventions Assist with patient assessments by observing and reporting patient status and responses Accurately document treatments, patient progress, and outcomes in the medical record Educate patients, families, and caregivers on exercises, safety, and functional techniques Collaborate with an interdisciplinary care team to support patient-centered goals Ensure treatments follow regulatory, ethical, and company quality standards Help maintain therapy equipment and a safe, organized treatment environment Participate in in service training, mentorship, and professional development activities Contribute to positive patient experience and support manageable, effective caseloads Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training Neuromuscular re-education Patient and family education Rehabilitation documentation and EMR use Knowledge of geriatric conditions and fall prevention Skilled nursing/post-acute rehab practices Infection control and safety procedures Interdisciplinary team collaboration
