Physical Therapist Assistant (PTA) Join Our Team at Rock River Nursing & Rehab in Fort Atkinson, Wisconsin We are seeking a dedicated and compassionate Physical Therapist Assistant (PTA) to join our team at Rock River Nursing & Rehab in Fort Atkinson, Wisconsin. As a PTA, you will play a vital role in providing high-quality physical therapy services to our patients, promoting independence, and improving overall well-being. About the
Role:
Provide physical therapy services to patients under the supervision of a licensed Physical Therapist, including exercises, stretching, and other modalities Assist patients in developing and achieving their individualized treatment plans Document patient progress and communicate with healthcare teams to ensure continuity of care Maintain accurate records and reports of patient treatments and progress Participate in patient education and discharge planning
Responsibilities:
Conduct patient evaluations and assessments to establish baseline data and identify areas for improvement Develop and implement treatment plans in collaboration with Physical Therapists and other healthcare professionals Implement and modify exercises, stretches, and other interventions as needed Monitor patient progress and adjust treatment plans accordingly Collaborate with Physical Therapists to ensure continuity of care and effective treatment
Requirements:
Associate's degree in Physical Therapist Assistant studies Current certification as a Physical Therapist Assistant (PTA) in the state of Wisconsin 1+ year of experience as a Physical Therapist Assistant in a skilled nursing setting Strong communication and interpersonal skills Ability to lift and transfer patients as necessary Familiarity with electronic medical records (EMRs)
