Job Description

Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support high-quality rehab services for seniors in skilled nursing and post-acute care settings. Working under the direction of a Physical Therapist, you will carry out treatment plans, lead therapeutic exercises, document patient progress, and help optimize functional outcomes and quality of life. You'll collaborate closely with an interdisciplinary team in a patient-centered, respectful environment that values integrity, clinical excellence, and open communication, with support for mentorship, continuing education, and manageable caseloads. Responsibilities Implement physical therapy treatment plans under PT supervision Assist with therapeutic exercises, gait training, and balance activities Monitor, document, and report patient progress and responses to treatment Educate patients and families on home exercises and safety Collaborate with PTs, nursing, and interdisciplinary team members Adhere to facility and regulatory documentation standards Ensure safe use of equipment and maintain treatment areas Support discharge planning and functional goal attainment Uphold patient-centered, ethical, and culturally sensitive care Participate in in-services, mentoring, and continuing education Required Skills Therapeutic exercise implementation Gait and balance training Transfer and mobility techniques Geriatric rehabilitation Skilled nursing/post-acute care knowledge Clinical documentation and EMRPatient and family education Functional outcome measurement Infection control and safety procedures Interdisciplinary team collaboration
