The Provider (Nurse Practitioner or Physician Assistant) will educate the patients on how to create a lifestyle of wellness that will ensure they have a long and productive life! This is a foundational approach to preventing and treating many chronic diseases. Truly help change lives! The Provider will work under the supervision of the medical director. Under general direction, will provide patient care; conduct physical exams; complete health history summary; document patients' progress and assists with patient care management.RESPONSIBILITIES
Obtain and review patient medical history information relevant to patient care Conduct physical examinations Order and review laboratory studies as well as diagnostic procedures Assume responsibility for prescribing FDA approved appetite suppressants according to state and federal guidelines Review patient progress and manage patient care accordingly Maintain all required documentation for patient care and medication management Conduct patient consultations Provide training to clinical and non-clinical staff as needed Document patient progress notes in the patient chart and/or EMR. Conduct consultations with patients regarding the program Perform other clinical duties as requiredQUALIFIED APPLICANTS MUST HAVE
Must have valid Medical & DEA License. Must ensure CAQH profile is current. Credentialed provider preferred. Must enjoy a team atmosphere and creating a supportive and encouraging environment for patients! Advanced Practice Registered Nurse (APRN) OR Physician Assistant - Certified Hormone replacement therapy (HRT) experience is a plus!Benefits:
