Job Summary We are seeking a dedicated and energetic Physician Assistant (PA) to join our dynamic PACE healthcare team. In this vital role, you will collaborate closely with physicians, nurses, and other healthcare professionals. THe Physician Assistant, will serve as a liaison between participants, community physicians both in the clinic and in the participants home. Duties Conducts comprehensive History and Physicals on new participants and current participants in any care setting (clinic, member's home in the community, nursing home, hospital, other care settings). Performs in-person comprehensive health histories and physical exams on new Albright LIFE participants. Coordinates with Interdisciplinary Team to develop a comprehensive plan of care for each participant. Conducts an in-person reassessment semi-annually and as needed. Integrates the primary care treatment plan into the overall plan of care developed by the Interdisciplinary Team. Interacts with team members to meet emergencies and acute need of participants. Participates in discharge planning for acute and long-term placement. Evaluates and treats participants during acute illness. Following consultation with the Primary Care Physician, refer participants to medical specialist as indicated. As directed by the Primary Care Physician, manages care of participants in the nursing home: Provides regular visits as dictated by nursing home standards and participant need. Performs telephone contact with nursing home staff as required. Maintains participants' medical record and fulfills Albright Life charting and reporting requirements. Provides relevant participant and caregiver education. Educates and trains participant and caregivers in appropriate care and interventions. Provides staff in-service on a variety of relevant topics. Completion and documentation of appropriate diagnostic coding. Orders, interprets, and follows up on lab tests and results. Performs selected procedures and documents orally and in writing. Initiates emergency measures as indicated. Recognizes abnormalities that justify immediate referral for intervention by others. Initiates, evaluates, reviews, and changes orders for selected medications and diagnostic studies/treatments within attending physician-approved protocols. Assesses, initiates, and coordinates participant care on a daily basis under the supervision of the Primary Care Physician. Request consultations from all services as needed in collaboration with Primary Care Physician and/or established protocols. Acts as liaison with physicians, nurses, and other health care professionals within the program, providing education, communication, and support for Albright LIFE staff, patients, and families. Directs patient management in conjunction with physician staff. Conducts periodic assessments. Advises the Medical Director and Primary Care Physician in ways and means to establish better accountability of Albright Life services to participants and referral sources, keeps Medical Director aware of needed material and human resources as program expands. Assists with the development of standards of care; performs on-going monitoring and evaluation of participant care practice and service delivery; provides guidance and training to staff regarding medical and quality assurance issues. Strong understanding of anatomy, physiology, pharmacology, and medical terminology relevant to acute care settings. Ability to work collaboratively within multidisciplinary teams under collaborative practice agreements; excellent client communication skills are essential. Critical care experience including ICU or trauma center exposure Additional skills such as telehealth experience, infectious disease management knowledge, pain management expertise, and geriatrics are desirable but not mandatory.

EDUCATION & EXPERIENCE REQUIRED

Bachelor's Degree from an accredited university. Graduate of certified or accredited Physician Assistant Program with at least two years related experience with three years' experience as Physician Assistant.

Licensure/Certification:

Current Cardiopulmonary Resuscitation (CPR) certification required. Current Physician Assistant license in Indiana required. Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact. Must have a valid driver's license. Must be able to meet workplaceauto insurance carrier's standards for coverage. Strong understanding of value-based care models, risk-based contracting, and regulatory compliance within Medicare/Medicaid frameworks. Excellent communication and presentation skills with diverse audiences. Exceptional customer service skills. Results-oriented; ability to handle multiple, time-sensitive projects and prioritize accordingly, while focusing on delivering required outcomes Strong communication and interpersonal skills written and oral; demonstrated ability to influence outcomes and work effectively with all organizational levels Commitment to demonstrating the highest standard of ethical conduct and integrity; strong willingness to continually embrace personal and professional development Deep commitment to serving older adults and vulnerable populations. Proficient with computers and Microsoft Office Willing to travel as needed.
