Provide patient evaluation, diagnosis, treatment, and documentation across all ages under supervision. Coordinate multidisciplinary services within a Medical Home model to optimize outcomes and cost-efficiency. Develop and monitor treatment plans integrating behavioral health, pharmacy, therapy, nutrition, or surgery, and refer to specialists as needed. Work at least 40 hours weekly with scheduled patient contact; participate in shared call rotations including evenings, weekends, and holidays.
