A Physician Assistant provides medical care to patients, under the supervision of a physician or surgeon. Works in all areas and specialties of medicine, and is licensed to practice medicine but has less formal certification than a physician. Performs duties delegated by a physician such as recording patient medical histories, ordering diagnostic tests, or giving immunizations. May serve as the primary healthcare provider, in consultation with a part-time physician, in communities with limited health care resources.

Job Description

We are looking for Nurse Practitioners and Physician Assistants to join our team in conducting patient centered care in patients' homes. The NP PA will be accountable for caring for patients, maintaining accurate and current patient records, and scheduling initial, urgent and follow-up appointments to patients as required. Monday - Friday Schedule Performs initial patient screenings/physical examinations/creates assessments plans Addresses urgent care needs of the patients as needed Orders, interprets, and analyzes diagnostic tests and determines the need for follow-up appointments and further treatment options. Prescribes and manage medications and treatments as appropriate, including but not limited to psychopharmacological agents. Deliver palliative care, and when needed, end of life care to patients and families. Delivers quality care while maintaining all company metrics pertaining to appropriate documentation.coding, HEDIS measure, engagement of patients and managing medical expense.
Qualifications:
Valid Nurse Practitioner or Physician Assistant license and Board Certification NPI number
BLS/ACLS
Valid DL, Auto Insurance, willingness to drive to patients homes patient location salary $ 101,000 - $ 156,000 depending on experience plus excellent benefits, 4 weeks PTO, annual bonus, and more Please apply with your resume and you will be contacted within 24 hours
