Job Description

Join a physician-led, value-based primary care organization dedicated to delivering high-quality, patient-centered care to adults and seniors in Green Valley, AZ. We are seeking a Board Eligible or Board Certified Family Medicine or Internal Medicine Physician to become part of a collaborative care team focused on improving health outcomes through preventive care, chronic disease management, and coordinated treatment. Practice alongside experienced Nurse Practitioners, Care Managers, Pharmacists, Social Workers, and Clinical Support Teams in a supportive environment designed to help physicians focus on delivering exceptional patient care.

Compensation & Benefits:

Key Responsibilities:
Deliver comprehensive primary care services for adult patients. Diagnose and manage acute, chronic, and complex medical conditions. Interpret diagnostic tests and coordinate specialty referrals when appropriate. Collaborate with interdisciplinary care teams to optimize patient outcomes. Promote preventive care, wellness, and chronic disease management. Participate in quality improvement initiatives and patient engagement programs. Maintain accurate and timely electronic medical record documentation.
Qualifications:
MD or DO from an accredited medical school Active Arizona medical license (or eligibility to obtain) Current DEA registration Board Certified or Board Eligible in Family Medicine or Internal Medicine Passion for delivering value-based, patient-centered care Excellent communication and collaborative care skills #AAS
