Job Description

We are seeking a Family Practice physician to join a collaborative, patient-focused care team in a modern outpatient clinic environment. This role offers the chance to provide comprehensive primary care while contributing to clinical quality initiatives and population health improvement efforts. The ideal physician is committed to relationship-based care, teamwork, and delivering an exceptional patient experience.
Key Responsibilities:
Provide direct primary care, including evaluation, treatment, and ongoing management of patients across the lifespan. Follow evidence-based standards of care while using data and chart review to guide clinical decisions. Work closely with nurses, care coordinators, and support staff through daily team huddles and shared clinical planning. Coordinate care with internal and community resources to ensure patients receive well-rounded support. Complete timely, accurate documentation in accordance with clinical protocols and compliance standards. Participate in clinical quality discussions, workflow improvements, and patient experience initiatives. Maintain active licensure and certifications while adhering to regulatory and accreditation requirements.
