Physician Assistant - Internal Medicine / Primary Care •High-Autonomy | Longitudinal Care | Physician-Led Practice •Artisan Primary Care is a growing physician-led practice providing longitudinal primary care services within assisted living, independent living, and memory care communities throughout the Phoenix area. We are seeking an experienced Physician Assistant who enjoys complex adult medicine, continuity of care, and practicing with a high level of autonomy in a collaborative physician-led environment. This role is heavily focused on: chronic disease management medication management continuity-based care reducing unnecessary hospitalizations coordination with families, specialists, hospice, and facility staff Providers in our practice care for the same patient panel over time and are expected to think longitudinally rather than episodically. Responsibilities Independently evaluate and manage medically complex adult and geriatric patients Diagnose and treat acute and chronic conditions Perform medication management and adjustments Review labs, imaging, and diagnostic studies Coordinate care with specialists, hospice, home health, families, and residential care staff Complete timely and accurate documentation Provide continuity-based primary care to established patients Preferred Experience 2+ years PA experience preferred Internal medicine Primary care Geriatrics Skilled nursing / residential care medicine Chronic disease management Qualifications Active Arizona PA license required Ability to start within approximately 3 months Strong clinical judgment and communication skills Comfortable practicing independently Experience managing medically complex patients preferred Compensation & Benefits $135,000-$156,000 annually depending on experience and fit Productivity incentives available Monthly vehicle stipend PTO and benefits package Physician-led collaborative environment This role is ideal for providers who value meaningful patient relationships, thoughtful chronic disease management, and long-term continuity of care.
