Job Description

Vivo HealthStaff is recruiting for Primary Care Physicians in Stockton, Modesto, Merced, and Indio, California. These are full-time permanent positions offering salary, benefits, and bonus opportunities. We are seeking compassionate and experienced Primary Care Physicians to provide comprehensive primary care to medically complex and aging patients in a clinic-based setting. The physician will work collaboratively with an interdisciplinary team to ensure coordinated, patient-centered care across home, clinic, and facility-based environments.

Full-Time, Monday to Friday; rotating night and weekend call

Permanent Salary:
$250,000 $280,000/year + bonus + equity
Key Responsibilities:
Provide direct medical care including assessments, diagnosis, treatment planning, and ongoing follow-up Collaborate with an interdisciplinary team to develop and update patient care plans Coordinate care transitions and discharge planning with hospital and skilled nursing facility staff Oversee medical care for patients in nursing homes and during hospital admissions Supervise and support nurse practitioners and other clinical staff Participate in 24-hour care planning and rotating call coverage
Requirements:
Medical degree from an accredited institution and completion of an ACGME-accredited residency program Board certification in Internal Medicine, Family Medicine, or Geriatrics (preferred) Active, unrestricted California medical license Active DEA and California CDS certification Eligible for Medicare and Medi-Cal provider enrollment Experience working with medically complex or elderly patient populations preferred
