Job Description
Job Overview We are seeking a dedicated Primary Care Physician specializing in Family Medicine or Internal Medicine to join our healthcare team. The physician will provide comprehensive, patient-centered medical care across diverse populations, emphasizing preventive health, chronic disease management, and acute illness treatment. This role offers an opportunity to collaborate with a multidisciplinary team to improve health outcomes and promote wellness in the community. The ideal candidate will possess strong clinical expertise, excellent communication skills, and a commitment to delivering high-quality patient care within a dynamic healthcare environment. Duties Conduct thorough patient assessments through detailed medical histories, physical examinations, and diagnostic evaluations. Develop and implement individualized care plans that incorporate evidence-based practices and patient preferences. Manage acute and chronic conditions across a broad spectrum of health issues, including diabetes, hypertension, respiratory illnesses, and infectious diseases. Utilize Electronic Health Record (EHR) systems such as Epic or similar platforms to document clinical encounters accurately. Coordinate with specialists and healthcare professionals across disciplines such as cardiology, pulmonology, dermatology, neurology, and more to ensure comprehensive patient management. Perform routine procedures including suturing, basic life support (BLS), immunizations, and wound care. Provide health coaching and preventive counseling on topics such as lifestyle modifications, smoking cessation, and nutrition. Participate in quality improvement initiatives aligned with healthcare standards such as NCQA or HIPAA compliance. Engage in outpatient clinic operations, triage assessments, and follow-up care to optimize patient outcomes. Support hospital or inpatient services when necessary, including post-discharge follow-up and perioperative care coordination. Requirements Graduate of an accredited school of medicine or osteopathic medicine with successful completion of an accredited residency in Family Medicine or Internal Medicine Current, unrestricted California medical license (MD or DO) and current DEA registration. Be willing and eligible to obtain a CA license. Board Certified or Board Eligible in Family Medicine, Internal Medicine, Pediatrics, or another closely related primary care specialty; candidates with a minimum of five (5) years of relevant clinical experience may also be considered. Recent experience in an outpatient/ambulatory primary care setting is preferred; inpatient experience is a plus. Current CPR or Advanced Life Support (ALS) certification. Proficiency with EMR/EHR systems, such as Epic or comparable platforms; familiarity with CPT/ICD medical coding preferred. Strong clinical knowledge and diagnostic skills, with the ability to evaluate and manage a broad range of primary care conditions and coordinate care with specialists, including cardiology, endocrinology, pulmonology, dermatology, and radiology. Excellent clinical judgment, communication, and patient-care skills, with the ability to establish effective relationships with diverse patient populations. Demonstrated ability to collaborate effectively with multidisciplinary healthcare teams. Knowledge of quality improvement initiatives, clinical research protocols, or standards such as NCQA is preferred. Commitment to evidence-based practice, ongoing professional development, HIPAA compliance, and patient privacy. Successful completion of required Occupational Health Screening. Must provide valid identification and employment authorization documentation as required by Form I-9 and complete employment verification through E-Verify.
