Geriatric Medicine Physician Newport Beach, CA Job Type Full-time Description Hoag Health, the top-ranked health system in Orange County, CA is seeking Geriatric Medicine Physician to join our growing program! Hoag Center for Brain Health, part of Hoag Health is a patient-centered medical center for geriatric patients. It is the continuity hub for every patient in the Center — providing longitudinal follow-up and comprehensive medical support, directing polypharmacy review and deprescribing, leading advanced care planning, and managing complex comorbidities across every clinical encounter. All specialist consultative services (behavioral neurology, geriatric psychiatry, palliative medicine, neuropsychology, occupational therapy, social work) report back into the single shared care plan that the Geriatric Physician would oversee.               Requirements:
              • M.D. or D.O. degree from a fully accredited medical or osteopathic school of medicine
              • Residency training and completion in Internal Medicine or Family Medicine
              • Board Certified in Internal Medicine or Family Medicine
              • Fellowship training in Geriatric medicine preferred
              • Active, unobstructed California medical license, or the ability to obtain one
              • Full prescriptive authority and an active DEA in California, or the ability to obtain one
              • Ability to successfully complete Hoag Health's credentialing process
