In this vital role, you will provide comprehensive, patient-centered medical care across a broad spectrum of health issues, emphasizing prevention, diagnosis, and treatment. Your expertise will help improve patient outcomes and promote overall wellness within our community. This position offers an exciting opportunity to collaborate with a multidisciplinary team and make a meaningful difference in patients' lives through proactive health management and personalized care. Duties Provide comprehensive, patient-centered outpatient primary care to patients across all age groups; no obstetrics (OB) . Conduct comprehensive physical examinations, review and interpret diagnostic results, and develop individualized treatment plans. Manage chronic conditions through patient education, preventive care, and guidance on self-management and healthy lifestyle practices. Coordinate specialty referrals, follow-up care, and inpatient services when medically necessary. Complete and manage medical forms and documentation, including DME, DMV, IHSS, EDD/disability, school, and other patient-related forms, as well as In-Basket management. Collaborate with clinical and non-clinical staff across multiple disciplines to deliver scheduled outpatient services while meeting established quality, productivity, and patient care standards. Partner with Behavioral Health and Wellness teams to integrate medical, behavioral health, and wellness services for AACI patients. Participate in clinical quality improvement initiatives, peer reviews, audits, certifications, and other quality and compliance activities. Serve on a rotating basis as an on-call physician to provide after-hours clinical support. Participate in community outreach activities, including health fairs and other community health events, as needed. Skills Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited medical institution. Board certification in Family Medicine, Internal Medicine, or a related specialty. Current, valid, and unrestricted California medical license. Active and unrestricted DEA license. Minimum of 2 years of clinical experience in primary care. Current BLS certification. Ability to manage a full-time patient panel of approximately 1,500 patients. Demonstrated commitment to serving underserved and multicultural communities. Ability to establish and maintain effective working relationships within a multidisciplinary team.
