Full-Time | Outpatient | No OB Federally Qualified Health Center (FQHC) serving diverse communities across Santa Clara County, is seeking a compassionate Primary Care Physician (MD/DO) to provide high-quality, culturally responsive outpatient care. Key Responsibilities Provide comprehensive primary care to patients of all ages. Perform examinations, interpret diagnostics, and develop individualized treatment plans. Manage chronic conditions and promote preventive care and patient education. Coordinate specialty referrals and hospital/inpatient care when needed. Collaborate with Behavioral Health, Wellness, and multidisciplinary care teams. Complete required medical forms and manage clinical in-basket responsibilities. Participate in quality improvement, peer review, and occasional after-hours call rotation. Support community health events and outreach activities. Qualifications MD or DO from an accredited medical school. Board Certified in Family Medicine, Internal Medicine, or related specialty . Active, unrestricted California medical license and DEA license . Minimum 2 years of primary care experience . Current BLS certification. Ability to manage a patient panel of approximately 1,500. Strong communication and teamwork skills. Commitment to serving underserved and multicultural communities. English proficiency required; additional languages are a plus.
