PAIN MANAGEMENT & INJURY RELIEF MEDICAL CENTER
Interventional Pain Management Physician Per Diem — Independent Contractor (1099) Thousand Oaks, California $3,000 per day 4 days per month Physician-owned pain management practice seeking a board-certified or board-eligible interventional pain physician for recurring monthly procedure coverage. The Role Four days per month, scheduled on consistent weekdays set in advance Procedures performed at our ambulatory surgery center and in-office procedure suite Full block of interventional pain cases: epidural steroid injections, medial branch blocks, radiofrequency ablation, joint and peripheral injections Dedicated medical assistant and procedure staff No call and no after-hours coverage Approximately $12,000 per month — $144,000 annually for 48 days of work Requirements Active, unrestricted California medical license and DEA registration Current California Fluoroscopy Supervisor and Operator Permit issued by
CDPH-RHB.
Physicians eligible for the one-time Temporary Fluoroscopy Permit are encouraged to apply. Board certified or board eligible in Pain Medicine (ABA, ABPMR, or ABPN), or Anesthesiology or PM&R with documented interventional experience Own professional liability coverage, minimum $1M / $3M, with our practice endorsed on the policy for vicarious liability. Certificate of insurance required before the first procedure day. If coverage is claims-made, tail coverage is the physician's responsibility. Willingness to complete credentialing with our contracted payors (we handle the paperwork) Compensation $3,000 per day, flat rate, paid as a 1099 independent contractor Same-day charting expected Twelve-month written agreement with a defined schedule set in advance To apply, send your CV and a brief note describing your interventional case volume to [Alison Hersel, ].
