Job Description

Join a close-knit healthcare team as a full-time Nurse Practitioner caring for adults and older adults with kidney disease and other complex chronic conditions. You'll split your time between two Southern California clinics, enjoy a predictable weekday schedule, and collaborate closely with physicians and an interdisciplinary team focused on high-quality, patient-centered care. Work schedule & locations Monday-Friday, 8:00 AM-5:00
PM Upland Care Center:
3 days/week
Apple Valley Care Center:
2 days/week Full-time role with consistent hours What you'll do Provide comprehensive clinical care for patients with kidney-related issues and multiple chronic conditions Perform assessments, develop and manage treatment plans, and monitor clinical progress Coordinate care across settings and work collaboratively with physicians and the care team Support adult and geriatric patients through ongoing management and education Compensation & benefits Competitive salary range: $119,313-$178,968 annually Comprehensive benefits package A collaborative, patient-focused work environment Who we're looking for Master of Science in Nursing required Active NP board certification Current, unrestricted California RN and NP licenses At least 2 years of NP experience in a relevant clinical area, or equivalent education/experience Specialty certification or a minimum of 1 year of nephrology experience required Strong clinical background in nephrology, primary care, or internal medicine and a passion for caring for adults and older adults Preferred qualifications Bilingual in English and Spanish Experience in primary care or internal medicine Prior experience working with adult and geriatric populations If you're an experienced NP who values teamwork, continuity of care, and working with complex patient populations, this is a rewarding opportunity to make a real difference in patients' lives.
