Primary Care Physician - Long Term Care / Skilled Nursing (Southeast Colorado) Senior Doc - 3.6 Pueblo, CO Job Details Part-time 21 hours ago Benefits Travel reimbursement Flexible schedule Qualifications DEA License Medical License Stakeholder relationship building Full Job Description Primary Care Physician - Long-Term Care & Skilled Nursing Flexible Schedule | Southeast Colorado (Lamar Trinidad) Lykos Medical as part of Brigade Health About the Role Lykos Medical as part of Brigade Health is seeking a Primary Care Physician (MD/DO) to provide supervisory and clinical oversight for Long-Term Care (LTC) and Skilled Nursing Facilities (SNF) across rural Southeast Colorado . This role is ideal for physicians seeking schedule flexibility , autonomy , and the opportunity to support underserved senior communities . What Makes This Role Unique Extremely Flexible Scheduling Minimum required in‑person rounding: once per month We schedule around your availability Ideal for physicians seeking additional income or flexible part-time work Travel-Friendly Compensation Productivity-based pay model Flat travel fee or stipend available due to long driving distances We collaborate with our physicians to maximize efficiency and route planning Leadership & Expansion Opportunities Candidates in Colorado Springs or Pueblo encouraged to apply May support future expansion into LTC/SNF markets in those areas Supported, Physician-Led Environment You'll have autonomy with operational support Strong facility partnerships already in place Clear expectations aligned with CDPHE & Medicare requirements Responsibilities Provide supervisory oversight and monthly in‑person rounding Manage chronic and complex geriatric patients in LTC/SNF settings Collaborate with facility nursing and administrative teams Document care in EMR in a timely manner Participate in quality improvement and post‑acute care initiatives Build strong relationships with rural facilities and care teams Qualifications Required MD or DO Board Certified/Eligible in Internal Medicine, Family Medicine, or Geriatrics Active CO medical license (or ability to obtain) DEA license Preferred Experience in LTC, SNF, geriatrics, post‑acute care, or rural medicine Familiarity with Medicare and value‑based care Interest in leadership or future program growth Who Thrives in This Role This position is ideal for physicians searching for: Primary Care Physician
