Full Job Description Salary Range:
$200,000.00 to $300,000.00 per year By adhering to Connecticut State Law, pay ranges are posted. The pay rate will vary based on various factors including but not limited to experience, skills, knowledge of position and comparison to others who are already in this role within the company. Physicians Alliance of Connecticut (PACT, LLC) has an Internal Medical Group with an opportunity for a Physician to work in Skilled Nursing Facilities in the Branford area. This is an excellent opportunity for those that enjoy a flexible work schedule, working autonomously, and accelerate in a pay for performance environment. The position would be full time working 40 hours per week. Summary Under indirect supervision, provides and manages health care for a specified patient population. Essential Functions Provides and manages direct patient care, including physical examinations, evaluations, assessments, diagnoses and treatment for a specified patient population. Engages patients and their families or caregivers in understanding, setting and monitoring self management care plans, and documents individualized care plans and care coordination in the patient chart. Analyze patients medical history, medication allergies, physical condition and examination to determine best care Diagnose medical condition Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions. Refers patients to specialists or to internal medicine as appropriate. Directs and coordinates the patient care activities of nursing and support staff as required. Respect and promote patient rights and privacy. Responding appropriately to emergency situations. Participate in meetings to evaluate practice processes and work flows pertaining to patient care and quality improvement efforts. Participate in specified health promotion, education and/or prevention programs. Participates in population health activities as directed by the practice. Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives , and safety, environmental, and/or infection control standards Skills and Knowledge Knowledge of legal and ethical standards for the delivery of medical care. Understanding of relevant drugs and non-pharmaceutical patient care aids and ability to prescribe dosage and instruct patients in correct usage. Knowledge of current principles, methods and procedures for the delivery of medical evaluations, diagnosis and treatments in area of expertise. Demonstrated skill in interviewing and patient coaching. EHR documentation as it pertains to patients and populations of patients. Education and Experience Medical Doctor with 3 to 5 years of direct patient experience which may include residency. Medical License, Board certification, Federal DEA certification
