2996669130-2 Apply for this job now Job Description Hartford Healthcare is seeking an Endocrinology Advanced Practice Provider to join our collaborative Healthcare & Medical Services team. In this role, you will evaluate and manage adults with diabetes and complex endocrine disorders in outpatient and hospital-affiliated settings. Responsibilities include comprehensive assessments, medication management, patient education, and coordination of multidisciplinary care. You will leverage cutting-edge technology and EMR tools, participate in quality initiatives, and contribute to a culture of compassion, teamwork, and clinical excellence. Responsibilities Provide outpatient and occasional inpatient endocrine evaluations and follow-up care Manage diabetes, thyroid, adrenal, and pituitary disorders using evidence-based protocols Interpret endocrine lab results and imaging and adjust treatment plans Educate patients and families on disease management, medications, and lifestyle changes Collaborate with endocrinologists, primary care, and multidisciplinary teams to coordinate care Document all encounters accurately in the EMR and ensure regulatory compliance Participate in quality improvement, clinical initiatives, and protocol development Precept or mentor staff and contribute to team education as needed Required Skills Endocrine disorder assessment and management Diabetes management and insulin titration Thyroid, adrenal, and pituitary disease management Interpretation of endocrine labs and imaging Chronic disease care planning Electronic Medical Record (EMR) documentation Patient and family education Interdisciplinary care coordination Quality improvement and outcomes tracking Clinical research or protocol adherence
