Work alongside a compassionate and collaborative interdisciplinary team that includes a full staff of nurses and support staff to support your patients Monday through Friday during business hours.
We provide:
Billing Free EMR Prior Authorizations Regular Financial Statements Marketing Support to grow your practice Training Access to DME and vaccines Credentialing Contracting with Insurances Team of Nurses to help you manage your patients Monday through Friday Roles and Responsibilities As a Physician in our home-based primary care team, you will: Conduct comprehensive health assessments and physical exams Develop individualized care plans tailored to each patient's unique needs and conditions Provide management of chronic diseases, such as diabetes, hypertension, heart disease, and COPD Prescribe medications, order diagnostic tests, and coordinate with other healthcare providers Educate patients and caregivers on managing health conditions and preventive care Collaborate with physicians, specialists, and allied health professionals to ensure seamless, holistic care for patients Maintain thorough documentation of patient visits and adhere to state and federal healthcare regulations
Qualifications Active Medical License:
Must hold a current and valid Medical license in your state without restrictions Active DEA Basic Life Support (BLS) Experience in
Primary or Geriatric Care:
A background in primary care, family medicine, geriatrics, or chronic disease management is preferred. Commitment to

Strong Communication Skills:
Ability to effectively communicate with patients, caregivers, and interdisciplinary team members.
