Heartline Staffing is recruiting a Primary Care Physician for an onsite opportunity in North Lauderdale, Florida, providing comprehensive, value-based primary care to senior patients. This physician will manage a panel of up to 450 patients while leading a dedicated care team and maintaining accountability for clinical outcomes, patient experience, and the detailed care of each patient.

POSITION OVERVIEW

This is an outcomes-focused primary care model centered on strong physician-patient relationships, health coaching, care coordination, and physician leadership. The PCP independently manages acute and complex chronic conditions common in older adults while working collaboratively across the care continuum.

PRACTICE MODEL

Patient Panel Of Up To 450 Patients 18-21 Appointment Slots Per Day New Patients Count As 2 Appointment Slots Follow-Up Patients Count As 1 Appointment Slot Value-Based, Outcomes-Focused Primary Care Dedicated Care Team Senior / Older Adult Patient Population

COMPENSATION
Base Salary Range:

$221,141

•315,915 Position May Also Be Eligible For Bonuses Or Commissions ROLE Independently Provide Care For Older Adult Patients With Acute And Chronic Illnesses Assess, Diagnose, Treat And Manage Acute And Complex Chronic Health Needs Maintain Accountability For Patient Care And Outcomes Build Trusting Relationships With Patients And Provide Ongoing Health Coaching Help Patients Establish And Work Toward Short-Term And Long-Term Health Goals Provide Patient Education, Health Promotion, Care Coordination And Documentation Lead A Care Team Including A Care Promoter, Care Facilitator And Care Coordinator Collaborate With Specialists While Remaining Accountable For The Detailed Care Of The Patient Engage With Hospitalists When Patients Are Hospitalized Coordinate With Transitional Care Teams For Patients In Hospitals, Skilled Nursing Facilities, Long-Term Care Or Homebound Settings Collaborate With Case Managers, Acute And Transitional-Care Physicians And Other Available Resources Participate In Risk Management, Quality Management And Clinical Meetings Help Cover Other Providers As Needed Participate In Center Management And Support A Positive Center Culture Supervise, Collaborate With Or Function Within A Practice / Collaborative Agreement With Advanced Practice Practitioners As Applicable Work Toward Clinical, Patient Experience, Quality And Medical Cost Performance Goals
