Physician - Post-Acute Care Inpatient Rehabilitation Facilities (IRF) & Skilled Nursing Facilities (SNF) Full-Time | Tampa Bay Area, Florida Join Inpatient Specialists Group At Inpatient Specialists Group (ISG), we believe in Synergy Empathy Continuity. ISG is seeking a full-time Post-Acute Care Physician to join our growing practice in the Tampa Bay area. This position provides an opportunity to practice across Inpatient Rehabilitation Facilities and Skilled Nursing Facilities throughout Hillsborough and Pinellas Counties The ideal candidate is an Internal Medicine or Family Medicine physician with an interest or experience in post-acute care, rehabilitation medicine, geriatric medicine, and the management of medically complex patients transitioning from the hospital setting. Position Overview The Post-Acute Care Physician is responsible for providing high-quality, evidence-based, and cost-effective medical care to patients receiving care in post-acute settings. The physician will work collaboratively with facility leadership, nursing, rehabilitation teams, specialists, primary care providers, advanced practice providers, and other members of the interdisciplinary care team to support clinical outcomes, continuity of care, and safe transitions throughout the patient's recovery. Key Responsibilities Provide comprehensive medical management of patients in assigned Inpatient Rehabilitation and Skilled Nursing Facilities. Perform initial evaluations, histories and physicals, follow-up visits, and medically necessary examinations. Diagnose and actively manage acute and chronic medical conditions. Develop and update individualized treatment plans based on each patient's clinical needs and goals of care. Order, review, and interpret laboratory testing, diagnostic studies, and other clinical evaluations. Prescribe and manage pharmacologic and non-pharmacologic therapies, including controlled medications when clinically appropriate. Participate in interdisciplinary and multidisciplinary rounds when applicable. Collaborate closely with nursing, rehabilitation therapy, case management, facility leadership, specialists, and other members of the care team. Coordinate specialty consultations and referrals when medically indicated. Facilitate appropriate escalation or transfer to a higher level of care when clinically necessary. Support effective transitions of care and communication with primary care physicians and other treating providers. Promote appropriate utilization of healthcare resources and adherence to clinical pathways, quality initiatives, and regulatory requirements. Maintain accurate, complete, and timely clinical documentation. Participate in shared evening and weekend call coverage as assigned. Qualifications Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO). Completion of an accredited residency program in Internal Medicine or Family Medicine . Board Certified or Board Eligible in Internal Medicine or Family Medicine. Active or eligible for an unrestricted Florida medical license. Active or eligible DEA registration. Current clinical knowledge and commitment to evidence-based patient care. Experience in post-acute care, skilled nursing, inpatient rehabilitation, geriatrics, or hospital medicine is preferred; however, physicians with a strong interest in developing expertise in post-acute medicine are encouraged to apply.
