Board Certified or Board Eligible in Pain Medicine (via Anesthesiology, PM&R, or Neurology subspecialty pathway). Active and unrestricted Indiana medical license (or eligible for state licensure within 90-120 days). Strong proficiency in interventional pain modalities, comprehensive patient assessment, and multidisciplinary care planning.
