Job Description

Specialty:
OB/GYN Type of Position:
Permanent, Hospital Employed Base Salary:
$450,000 base annual salary + % of gross charges Sign on bonus: $50,000 Relocation assistance: Up to $10,000 Retention bonuses/Student loan repayment
Shift/Work Schedule:
Full time, 4 days a week (if on call for the weekend, the providers typically work .5 day on Friday)
Work Hours:
4 days a week (combination of clinic and surgery blocks)
Call Schedule:
1 in 4 call for weekends (we have a locums provider that covers every 4th weekend/some holidays), 1 in 3 call throughout the week. Must be within 30 mins of hospital.

Inpatient/Outpatient Clinic Volume:
Average of 20-25 patients per day.
Procedures Required:
TVT sling, Laparoscopic sacrocolpopexy, dilation & curettage, total laparoscopic hysterectomy, endometrial ablation, hysterectomy, LEEP, laparotomy, Myomectomy, laparoscopic oophorectomy, salpingectomy, tubal ligation
Support Staff:
Each provider has their own
MA Credentialing Time Frame:

Credentialing and Licensing Requirements:
BC/BE, Indiana License Willing to accept new grad or Board Eligible candidate?: Yes Willing to wait for State Medical License? Yes Willing to accept J1 or H1 visa candidates?:
No Benefits Offered:
(Health, Retirement, CME, Relocation Etc): All premiums paid by hospital for employee and family (health/dental/vision) plus we contribute $1,630 annually to an HSA (if on hospital's health plan).
Pension plan for retirement:
Will match up to 3% of gross wages. Towards the end of year, we will pay an additional amount equal to 9.7% of earnings which can be deferred to a pre-tax 403b, 457b account or an after-tax
IRA. CME
$5000.
Subscription and Dues:
$1500. Cell phone allowance $1080.
Residency Stipend:
We will pay a residency a month stipend if they sign with us •Facility does not have a robot, so provider would need to be able to perform laparoscopic surgery.
