Geriatrics Opening Attleboro, MAFull TimePermanent PositionSeeking BC/BE Geriatric Medicine Physician (or IM/FM with geriatrics expertise) to join team in
Attleboro, MassachusettsPosition Highlights:
Split schedule with 50% clinic and 50% community-based careCoverage across Bristol and Norfolk CountiesTravel required for home and community visitsNo or limited call responsibilitiesFocus on complex geriatric population and chronic disease managementCollaborative interdisciplinary care modelOpportunity to incorporate telehealth and remote monitoring
Responsibilities:
Provide geriatric care in clinic and community settingsManage complex medical conditions in older adultsCoordinate care with interdisciplinary teamPerform home visits and community-based evaluationsDevelop and manage individualized care plansParticipate in care transitions and population health initiatives
Qualifications:
Board Eligible or Board Certified in Geriatric Medicine or Internal Medicine/Family Medicine with geriatrics experienceActive Massachusetts license and DEA or ability to obtainExperience with complex geriatric patients preferredValid driver's license and ability to travel required
