Job Description

Job Overview We are seeking dedicated and skilled Palliative Care NP's to join our healthcare team. The ideal candidates will possess a strong background in Palliative Care and demonstrate expertise in patient care, clinical research, and the treatment of chronic or life limiting illnesses. Key Responsibilities Conduct comprehensive patient assessments and develop individualized care plans. Manage and administer medications for pain and symptom relief, including both pharmacological and non-pharmacological interventions. Collaborate with multidisciplinary teams, including physicians, social workers, and other specialists, to ensure coordinated care. Facilitate advance care planning and discussions about goals of care with patients and families. Monitor and manage complex symptoms, escalating care to physicians or other healthcare professionals when necessary. Provide education and guidance to patients, families, and staff on palliative care principles and best practices. Maintain accurate documentation and comply with State, federal and facility policies, infection control, and quality improvement initiatives.
Collaboration:
Ability to work closely with other healthcare professionals, including primary care physicians and specialists, to ensure comprehensive patient care
Requirements:
Advanced nursing degree (Master's or higher) and NP certification. Previous Clinical experience in palliative care, hospice, oncology, geriatrics, or related fields. Strong understanding of medical terminology, coding (CPT, ICD), and EHR documentation practices. Experience with critical care, including ICU experience, is preferred. Knowledge of related specialties such as endocrinology, cardiology, gastroenterology, and urology is advantageous. Excellent communication skills for effective patient interaction and collaboration with healthcare teams. Ability to perform basic life support and manage acute pain effectively.
