• Active Michigan license and Controlled Substance Registration (CSR) are mandatory upon submission; no exceptions.

• Local Candidates:

• Preference for candidates who can drive in.

• Background Check:

• Clean record with no actions; minimal malpractice history is acceptable.

• Experience:

• Mandatory prior experience (Completion of Fellowship with at least 2-3 years in practice) is non-negotiable.
