$250,000 - $325,000 Annually Practice Medicine Beyond the Clinic Walls You didn't spend years becoming a physician to practice medicine five minutes at a time. At Twin Cities Physicians, you'll build a consistent patient population, manage complex older adults longitudinally, and bring high-quality primary care directly into the senior living communities where your patients live . It's a different model of medicine. Instead of a waiting room and an endless stream of unfamiliar appointments, you'll round within assigned communities, develop relationships with patients and families, and collaborate directly with the nurses and care teams who see your patients every day. A Different Primary Care Model Twin Cities Physicians is a growing, physician-led medical group serving more than 4,500 seniors across 350+ senior living communities throughout Minnesota . Our physicians care for older adults in assisted living, memory care, independent living, and long-term care settings, managing everything from preventative medicine and chronic disease to acute changes in condition and transitions following hospitalization. Because you're following patients over time, you know their baseline. You understand their medical history. And when something changes, you have the context to make informed clinical decisions earlier. You'll also have experienced Nurse Practitioners and Physician Assistants working alongside you , allowing you to practice within a collaborative medical group rather than carrying the clinical responsibility alone. Why Physicians Consider Twin Cities Physicians $250,000 - $325,000 annual compensation Weekday-focused rounding model Build meaningful longitudinal patient relationships Greater autonomy over how you manage your patient population No traditional clinic waiting room or high-volume office schedule Experienced NP and PA support Collaborative, physician-led medical group Practice proactive medicine focused on the needs of medically complex older adults Opportunity to improve continuity and reduce avoidable hospitalizations Established and growing presence throughout Minnesota What You'll Do Conduct on-site medical rounds within assigned senior living communities Provide comprehensive primary care for older and medically complex adults Diagnose and manage acute and chronic medical conditions Oversee chronic disease management and preventative care Coordinate care following hospitalizations and specialty consultations Partner with NPs, PAs, nurses, pharmacists, therapists, and senior living teams Lead care planning conversations with patients and families Recognize and respond to changes in patient condition Maintain accurate and timely clinical documentation Help advance a model of proactive, community-based medicine What We're Looking For Active Minnesota physician license in good standing MD or DO Board certified or board eligible in Family Medicine, Internal Medicine, or Geriatrics Strong clinical judgment and comfort managing medically complex older adults Excellent communication and interdisciplinary collaboration skills Valid driver's license and reliable transportation for community-based travel Experience in primary care, geriatrics, senior care, post-acute care, skilled nursing, or other settings serving medically complex adults is preferred. You'll travel between communities, work with interdisciplinary teams, and care for patients in the place they call home.
