Job Description

We are seeking a BoardCertified or Board Eligible Family Medicine or Internal Medicine Physician tojoin a rural primary care practice in Colebrook, New Hampshire. This is anexcellent opportunity to provide comprehensive care to patients in aclose-knit, underserved community while enjoying a balanced lifestyle andstrong community connection. Position Highlights Rural outpatient primary care Comprehensive care for adults and/or patients of all ages Manageable patient panel with opportunity to practice autonomously No weekend or holiday shifts Collaborative, supportive medical team Opportunities for quality improvement and community health initiatives Responsibilities Provide comprehensive primary care, including physical exams, diagnosis, and treatment. Manage acute and chronic medical conditions and develop individualized care plans. Provide preventive care, wellness visits, immunizations, and health screenings. Order and follow up on diagnostic testing and communicate results to patients. Prescribe and manage medications and treatment plans. Coordinate referrals and care with specialists and other healthcare providers. Educate patients and families on disease prevention, nutrition, lifestyle changes, and healthy living. Collaborate with nurses, medical assistants, and other members of the healthcare team. Maintain accurate and timely medical records. Participate in quality improvement initiatives.
