The Patient Navigator, Neurology Outpatient plays a central role in optimizing the patient journey and driving positive outcomes. By acting as a dedicated advocate, the Navigator streamlines the care process by eliminating barriers, facilitating communication between patients and providers, and empowering patients with the resources to manage their treatment confidently. This role is crucial for enhancing the patient experience, improving clinical outcomes, and enhancing overall practice efficiency. Under the direction of practice leadership, the Navigator provides key support to the clinical team, ensuring a smooth, coordinated workflow and greater patient satisfaction.
Qualifications:
Education, Knowledge, Skills and Abilities Required:
Minimum of a Bachelor's Degree. Minimum 2 years as a Registered Nurse. Proficient Computer Skills that may include, but are not limited to, Google Suite Platforms. Excellent written and verbal communication skills. Strong organizational and critical thinking skills. Must be organized and have the ability to multitask, working in a fast-paced environment. Must have the ability to work independently, but also in a team environment.Education, Knowledge, Skills and Abilities Preferred:
Experience with EPIC record system.Licenses and Certifications Required:
