Job Description

A physician practice group is seeking full-time physicians for opportunities in skilled nursing facilities and other post-acute care settings. We have opportunities in multiple locations, including Kalamazoo, Michigan; Cortland and Syracuse, New York; Corpus Christi and San Antonio, Texas; Whiteville, North Carolina; Knoxville, Tennessee; Hartford, Connecticut; Worcester, Massachusetts; Middletown, Delaware; and more. These roles offer a high degree of independence, flexibility, and the opportunity to build lasting relationships with patients, families, and facility teams. They are a strong fit for physicians who want meaningful clinical work, better balance, and the potential to take on leadership responsibilities over time. Physicians will provide ongoing primary care for patients in both short-term rehabilitation and long-term care settings. Responsibilities include performing assessments, managing acute and chronic medical conditions, developing and updating care plans, ordering and interpreting labs, prescribing and adjusting medications, coordinating closely with nursing and interdisciplinary teams, communicating with patients and families, and completing clear, timely documentation. Some positions also include medical director responsibilities. These roles include an additional stipend. The group provides strong clinical infrastructure, operational support, structured onboarding and training, and opportunities for advancement as its post-acute care programs continue to grow. Compensation varies by location but generally includes a base salary of approximately $250,000 to $300,000 or more, with unlimited bonus potential. Total earnings increase based on productivity, and medical directorship roles include an additional stipend.
