$270,000-$320,000 annually, based on experience A mission-driven community health organization is seeking a Family Medicine Physician to join its growing primary care team in Middletown, NY. Responsibilities Provide comprehensive outpatient primary care to patients across the lifespan. Diagnose, treat, and help prevent acute and chronic medical conditions. Promote preventative care, health education, and long-term wellness. Collaborate with physicians, advanced practice providers, behavioral health clinicians, nursing staff, and support teams. Maintain accurate, timely clinical documentation. Deliver culturally responsive, patient-centered care to a diverse community. Qualifications MD or DO from an accredited medical school. Board Certified or Board Eligible in Family Medicine. Active New York State medical license or eligibility to obtain one. Active DEA registration. Current BLS certification. Strong commitment to community-based care and serving diverse patient populations. Spanish-speaking ability is a plus.
