Competitive, based on experience and autonomy (discussed at interview) About the Role Medley Health is looking for a compassionate Physician or Nurse Practitioner for a part-time opportunity providing on-site physiatry and rehabilitation services across subacute rehab facilities. You'll work independently with strong administrative support, collaborating with interdisciplinary teams to help patients regain function, improve mobility, and manage pain as part of a broader rehabilitation-focused approach. What You'll Do Perform comprehensive physiatry and rehabilitation evaluations and follow-up visits in subacute rehab settings Diagnose and manage musculoskeletal, neurologic, and functional limitations in a subacute rehab population Develop individualized treatment plans focused on functional restoration, mobility, and pain management Recommend and manage medications, including controlled substances, as part of an overall rehabilitation and pain management plan Administer therapeutic injections, such as joint and trigger-point injections, to support pain relief and functional progress Coordinate with PT, OT, and ST teams on plans of care Document assessments and progress notes accurately and timely Communicate recommendations to facility clinical teams and assist with care transitions as needed What We Offer Part-time schedule with a high level of autonomy No nights, no weekends Malpractice insurance coverage (as applicable) Laptop provided Strong administrative support and streamlined scheduling Collaborative environment with nursing and therapy teams Qualifications - Physician (MD/DO) MD or DO; Board Certified or Board Eligible in PM&R preferred Active New York medical license Active DEA license required Experience in subacute rehab, geriatric care, rehab medicine, or related settings required Strong clinical judgment, documentation, and communication skills Qualifications - Nurse Practitioner NP license (Adult-Gerontology preferred; FNP considered with relevant experience) Active New York NP license Active DEA license required Experience in subacute rehab, geriatrics, orthopedics, or internal medicine required Ability to work autonomously and manage a patient panel Strong documentation and communication skills Why Medley Health?
